How to enter

  1. Pick the category you want to make a nomination for and tick the box next to it.
  2. Complete section 1 with the details of the person/team you wish to nominate.
  3. Complete section 2 with your own details in case we need to contact you.
  4. Complete section 3 with your story. No more than 250 words please.
  5. Submit the form.
NB – The closing date for nomination is 5pm on Monday 7th September 2009. We regret that forms received after this date will not be submitted to the judging panel.

Please select the box next to the category you wish to nominate. You may nominate an individual or team in more than one category however the event organisers reserve the right to assign nominations to a category they believe is more suitable to the nomination.
